What is In Vitro Fertilisation or IVF Treatment?

Normally, or through natural conception, the sperm fertilises the inside a woman's body. If the fertilised egg attaches to the lining of the womb and continues to grow, a baby is born about 9 months later.

In Vitro Fertilisation or IVF is a form of assisted reproductive technology (ART) available to couples who are unable to conceive either naturally, or when preliminary fertility treatments fail. ‘In vitro’ means ‘outside the body’.It is carried out using advanced clinical and laboratory techniques.

IVF involves 5 basic steps:

  • varian Stimulation using injections and ultrasound monitoring
  • Egg retrieval
  • Insemination leading to fertilisation
  • Embryo culture
  • Embryo transfer (Fresh/Frozen)

When IVF is recommended and for whom?

IVF is an individualised treatment plan advised by the doctor, based on patient characteristics like age, cause of subfertility, the egg number in the ovaries, male semen parameters and the previous fertility treatments received by the couple.

Main indications for IVF include: Blocked tubes, abnormal sperm parameters including azoospermia, poor ovarian reserve, advanced maternal age, recurrent pregnancy loss, unexplained infertility, failure to conceive with first- and second-line fertility treatments

IVF Procedure in our hospital, chennai

The following steps detail the IVF procedure, which usually spans 2 to 3 weeks.

Ovarian Stimulation: Ovarian stimulation involves stimulating the ovaries with injectable fertility drugs. Without them, your body will typically mature one (or maybe 2) eggs each cycle. For IVF, we require multiple eggs, as not all eggs will fertilise or show normal growth as an embryo. The number retrieved largely depends on the age and ovarian reserve of the patient.The first day of the IVF cycle is when you get your period. Before starting the fertility drugs, an ultrasound is done to check the uterine lining and the ovaries for follicles/cysts and a baseline hormonal profile is done to rule out any cycle disturbances. Once the injections are commenced, cycle monitoring starts to track follicular growth and to adjust injection dosages. Once 3 or more follicles reach a size more than 17mm, a decision for triggering the maturation of eggs (one time injection). Egg retrieval is planned 35-36 hours after the trigger injection.

Egg retrieval: Egg (oocyte) collection is a day care procedure. The patient is admitted to the hospital on the day of collection in the morning. The procedure is carried out under sedation/anaesthesiato avoid discomfort,and it typically lasts 15 to 30 minutes. The eggs are retrievedfrom the follicles under vaginal ultrasound guidance,using anaspiration needle into sterile tubes and transferred to the embryology lab. Generally, most of the follicles will carry a mature, healthy egg. Occasionally immature eggs are encountered. It is very rare that no eggs are found. Post procedure you will under clinical observation in the clinic for a few hours and will be discharged the same day itself if all is well.

Insemination: In this step, the eggs and sperms are brought together to effect fertilisation. The lab examines andprepares the eggs under the microscope. Only mature eggs can be used for insemination.In IVF, the eggs are placed in a petri dish and processed sperms are added to the dish for fertilisation (union) on their own. In partners with abnormal semen parameters, the spermatozoa need extra help with the fertilisation process. In such cases, intracytoplasmic sperm injection (ICSI), may be employed wherein a single sperm is directly injected into an egg.

Embryo culture: Most of the mature eggs will become embryos after fertilisation. The embryos are grown in a culture dish placed inside incubators and their growth is observed till day 5. The embryos are graded based on the appearance and stage and, subsequently, the best grade embryo is replaced into the uterus in the same cycle (fresh transfer) or may be frozen for use in a subsequent cycle.Usually, one to 2 of these healthy embryos will be transferred.

IVF: Side effects and risks:

IVF is generally considered safe.As with any medical procedure, there are some associated risks. Some of the side effects may include the following:

Injection related: Some patients may experience headache, nausea, fatigue, mood swings, bloating, soreness at injection site or breast heaviness during the stimulation phase. These symptoms are usually hormone-related and are tolerable and self-limiting.

Egg retrieval related: Mild cramping, spotting and abdominal discomfort may affect some women after the procedure. This is easily controlled by post procedure pain medications.

Ovarian hyperstimulation syndrome (OHSS): This occurs in less than10% of women undergoing the procedure due to hyperstimulation. Symptoms include nausea, vomiting, abdominal pain and distension. These symptoms are seen around 3 to 7 days after egg collection. These are usually mild in most of them and early improvement with close outpatient monitoring and supportive care is the norm. In 1-2% cases, OHSS can be more severe and may require hospitalisation.

Birth defects in baby: The risk of birth defects in babies born out of IVF is very low and is comparable to the general population risk.
